Care Coordinator

The Opportunity

FCA is excited to announce an opportunity for the position of Care Coordinator

The Care Coordinator will explore Social Determinants of Health barriers to care and provide case management, health education, and referrals and follow-up for individuals receiving services in the Family Treatment and Recovery Centers. The Care Coordinator will communicate with outside physical and mental health providers and assist on-site providers with Medication Assisted Treatment. The Care Coordinator will provide Targeted Case Management as a means of increasing participant access to various resources and services.

Responsibilities

  • Ensure same day availability is offered for those seeking Medication Assisted Treatment services and obtaining any needed registration information for appointment
  • Complete Social Determinants of Health screenings and provide referrals to assist with barriers and priority needs including transportation services for on-site appointments
  • Monitor client progress towards improving access to care through follow-up with participant and service providers as needed
  • Assure the provision of education, risk reduction, counseling and referral services to all clients regarding HIV/AIDS, tuberculosis, hepatitis, sexually transmitted diseases, and other communicable diseases
  • Ensure entitlements, insurance, and benefits are in place
  • Enter clinical data into electronic systems accurately & efficiently
  • Complete all documentation in a timely, client-centered manner, completing all documentation in a timely, client-centered manner
  • Provide referral to services that best meet client needs; advocate as needed for appropriate service provision; and follow up on outcome of referral, obtaining all necessary information from referred provider
  • Assist Medical Providers in the coordination of MAT services
  • Conduct client and community outreach as needed


Qualifications

  • Bachelor’s Degree in Social Work or related human services field 
  • Minimum of one year of experience in the health or human service field (behavioral health or substance use treatment preferred).
  • Intermediate computer skills
  • Excellent verbal and written communication skills
  • Valid and clean Driver's License

  • Salary:23/hr
  • This role is full-time (35 hours/week), Nonexempt
