In Home Counselor

The Opportunity

Family and Children's Association is seeking Full-Time and Per Diem In-Home Counselors for our Children's Mental Health Division. The In-Home Counselor provides individual, family, and group services under Community Psychiatric Supports and Treatment to children and their families who are diagnosed or at risk-of being diagnosed with a mental health disorder. Services are provided in the client's home and/or community and work to address challenges associated with a behavioral health need.

Responsibilities

  • Provide in-person services to children and their families in their homes and communities to help improve child's functioning and management of behaviors and symptoms.
  • Complete progress notes for each session and telephone calls.
  • Complete treatment plans in accordance with State and Managed Care Organization Guidelines.
  • Coordinate with child's Care Managers or other providers, when needed.
  • Submit work by deadlines and in compliance with funding regulations and the agency's Compliance Plan.
  • Deliver and document services using a strength-based and family-driven approach.
  • Assume other responsibilities assigned to support the operations of the program.

Qualifications

  • MSW or related degree, at least 1 year experience working with children with an emotional disturbance.
  • Knowledge of community resources and mental health diagnoses.
  • Valid New York State driver's license with a safe and responsible driving history.

Additional Information

Salary- Full Time Non Exempt: $45,000

Location: Garden City

Schedule: F/T, Evenings Required
