Program Summary:

EISEP provides crisis intervention, case management and in-home services for isolated homebound seniors ages 60 and up. They connect seniors to available community resources that will help them remain safely and independently at home.

Primary Responsibilities include:

Wellness visits by interns support the goal of keeping clients safe in the community and improving overall quality of life. They will be responsible for providing supportive in-home assessment and counseling to isolated homebound seniors. It will involve intensive case management, including linking clients to long term care resources and services, coordination with the senior team, and community outreach. Interns will be required to conduct home visits. All clients reside in Nassau County.

Qualifications and Requirements include:

  • 2nd Year Master of Social Work (MSW) Student
  • Valid New York State driver's license with a safe and responsible driving history to conduct home visits
  • Vehicle that is properly registered and insured by New York State and is available for use to conduct home visits

  • Monday thru Friday between 9am - 5pm
  • Blended Work Assignment (Both Remote & On-Site Work)

  • LMSW, SIFI Certified
