Social Worker $32.97/hr

The Social Worker will Supervise the therapeutic milieu of our Lakeview House Community Residence in coordination with the Program Coordinator to ensure the effective and efficient delivery of service to clients as defined by agency policies and procedures.


Primary Responsibilities include:

  • Create, update and manage treatment plans.
  • Trains families in the use of The Behavioral Management Plan.
  • Conduct family engagement events to foster positive relationships with parents and residents.
  • Complete required paperwork including comprehensive Medicaid billable, case notes, treatment planning, logs, CAIRS, NIMIRIS, incident reporting and Justice Center Reports.
  •  Role modeling intervention skills in the milieu to staff and residents.
  •  Communicate with outside providers regarding the overall clinical treatment of residents.
  • Communicate with School districts regarding the education plans for each resident.
  • Responsible for the orientation of new residents.
  • Meet with the program Coordinator on a regular basis to discuss resident's overall progress. 
  • Work as part of the interdisciplinary team at Lakeview House
  •  Supervise and guide bi-weekly treatment team meetings.
  • Makes necessary referrals to agencies that may assist residents and families with needed services.
  •  Identify and schedule workshops for families.
  • Must be On call for emergencies.
  • Follow discharge youth on aftercare, maintaining at minimum monthly contacts for a period of 3 months post discharge.
  •  Additional tasks as requested.

Qualifications and Requirements Include:

  • Master's Degree required.
  • Two years relevant experience with population and/or residential services. 
  • One year working with mental health/families preferred.
  • A valid New York State driver's license with a safe and responsible driving history.  Own motor vehicle properly insured and registered in accordance with NYS DMV regulations, and able to drive to client homes, meetings, and other worksites.
  •  Proficient in MS Office (Word, Excel, Outlook).

Schedule:

Full Time, 35 hours per week. Some night and weekend hours required.

Hours may vary depending on family and house needs.
