Case Manager

The Opportunity


FCA is seeking a Case Manger for our EISEP Program, which provides case management assistance and monitoring of homecare for individuals 60 years of age and older.  The Case Manager connects seniors to available community resources that will help them remain safely and independently at home.


Responsibilities

  • Conduct initial needs assessment and develop care plan for needed services.
  • Monitor and re-assess open cases to assure that all services are being delivered as planned.
  • Provide advocacy for clients regarding entitlements and other basic needs.
  • Provide representation at DSS, SSA etc., when clients are unable to represent themselves.
  • Visit clients in their homes as per EISEP protocols

Qualifications:

  • Bachelor's degree in social work or related field.
  • Experience with the geriatric population preferred.
  • Excellent verbal and written communication skills required.
  • Proficient Computer skills with MS Office.
  • Valid and clean driver's license required. 



Compensation Information

  • The competitive compensation range is reflected with FCA's framework, culture, and values. Individual pay decisions will vary based on skills and experience' while still allowing for growth.


Additional Information

  • Salary: $26.21-28/hr
  • Schedule: Monday-Friday, 9am-5pm, hybrid model.
  • This role is full-time (35 hours/week), nonexempt
