Conduct field-based navigation and linkage to care to increase community knowledge of HIV and STI's. decrease the risk of acquiring HIV infection, improve health outcomes for people living with HIV/AIDS, and prevent transmission of HIV to their sexual or substance use partners. In addition, facilitate educational workshops promote successful long-term recovery and address intersections between mental health, sexual health and substance use. Seeking Safety groups are geared towards addressing the impact of past trauma and are delivered to community members in various settings. Conduct outreach to promote program operations and services and facilitate working relationships with community partners.
Qualifications and Requirements include:
· High school diploma or general education degree (GED) with at least 6 months related experience and/or training; or equivalent combination of education and experience.
· A valid New York State driver's license with a safe and responsible driving history.
Schedule:
· F/T 35 hours per week, some evenings and weekends required.
