Psychiatric Rehabilitation Services Provider

The Opportunity

FCA is seeking a Psychiatric Rehabilitative Services Provider(PRS) to help individuals in behavioral health recovery choose, get, and keep life roles that are meaningful to them. PRS providers strive to enrich and empower the lives of individuals with psychiatric and substance use disorders to develop skills and enhance functioning to maintain community living; support positive social, emotional, and educational development; facilitate inclusion and integration; and support pursuit of goals in the community.

Responsibilities

  • Provides and facilitates an individual's skill development and support which includes daily living, socialization, educational/vocational supports and opportunities, and leisure/recreation activities planning to include community integration and community connections in person or offsite in the community
  • Provide crisis intervention to assist with deescalating situations.
  • Participate in integrated comprehensive person-centered and family centered family diagnostic and treatment planning, in accordance with clinic standards for OASAS, OMH and as a Certified Community Behavioral Health Clinic (CCBHC)
  • Monitor treatment plans and service delivery, in conjunction with interdisciplinary team, to resolve barriers to service utilization
  • Expand skills through ongoing education, collecting resources, developing support strategies, and attaining employment of their choice using the principles of IPS (Individual Placement & Support)
  • Provide education & coaching for the participants and their support system regarding medications, health behavior changes, and accessing community resources & services.
  • Function as an integral member of the inter-disciplinary treatment team and participate in case conferences, team meetings, in-service workshops, and trainings
  • Document data, service provision, and statistical reporting accurately, and efficiently into respective electronic systems and in compliance with governing oversight agencies.
  • All other duties as assigned by the director and direct supervisor.
  • High School Diploma required with 1-3 years of experience with career counseling of clients with alcohol/substance abuse disorders and/or clients from other disadvantaged populations; bachelor's degree preferred
  • Bilingual in Spanish and English preferred

  • Salary: $27.47/Hour
  • This role is full-time (35 hours/week), nonexempt
