Care Coordinator

Location: Bohemia, NY | Schedule: Full time 35/hours | Status: Non-Exempt |Salary Range: $24.73/hour

The Opportunity

Family and Children’s Association (FCA) is a Care Coordinator for our Sherpa program, a program that offers guidance to individuals and families navigating the substance use disorder (SUD) treatment system.

Responsibilities

  • Identify priority needs and potential barriers to care with the objective of improving access to services and promoting positive health outcomes.
  • Provide client-centered education, risk reduction strategies, and case management related to substance use, mental health, and physical health services.
  • Conduct screenings for HIV, Hepatitis C, and STIs among at-risk populations, and facilitate timely linkage to care for individuals who test positive
  • Track participant progress and service engagement through ongoing communication with clients and coordination with service providers.
  • Maintain comprehensive, accurate, and up-to-date client records, and ensure timely data entry into internal systems to support monthly and annual reporting requirements.
  • Coordinate care for clients with complex needs by managing referrals, securing follow-up information, and ensuring continuity of care across service systems.

Qualifications

  • High school diploma or GED required; Bachelor’s degree preferred.
  • A minimum of 1–3 years of experience in case management is required.
  • Prior experience working with individuals affected by substance use disorders is highly desirable.
  • Proficiency in Microsoft Word and database management systems is essential.
  • Valid driver’s license with a demonstrated record of safe and responsible driving.
