Outreach Specialist

Primary Responsibilities Include:

·  Provide outreach to individuals and families struggling with behavioral health challenges to facilitate their engagement in services and offer additional support and case management through the SSN program.

·  Act as a resource to all SSN staff members on issues related to substance abuse, mental health, and resources/services/connections available in the community.

·  Provides telephonic, virtual, and face to face outreach, engagement, trainings, referrals, and service planning in senior centers, libraries, senior housing programs, physician's offices and local health care providers and community settings as needed.

·  Support agency efforts to reduce stigma and educate community members on mental health and substance use issues and services; become certified in Mental Health First Aid and work in conjunction with the Director of MHAT (mental health awareness training) program to offer community education programs.

·  Act as liaison between the FCA SSN program and the communities it serves to establish and build new relationships and foster existing relationships with community stakeholders, treatment providers, and other community resources.

·  Assist SSN peers in providing individuals and families social and recovery support services, counseling, referrals, and linkages to prevention, treatment, and harm reduction services.

·  Work with FCA Development team to launch a public health awareness campaign on mental health and SUD in older adults.

Assume other duties as assigned. 

Qualifications and Requirements include:

  • Master's Degree required
  • QHP credential required: LCSW, LMSW, LMFT, LMHC
  • 2-5 years' experience providing counseling services
  • Strong communication and rapport-building skills, and the ability to engage a diverse population of clients required

  •  Ability to maintain appropriate professional boundaries and demeanor even when confronted with emotional or volatile situations.
  • Proficiency with Microsoft Office required
  • A valid New York State driver's license is required.

Schedule:

·F/T 35 hours per week
