Primary Responsibilities include:
in-home initial assessments for caregivers in need of supportive services.
short term, in-home individual counseling for caregivers.
organize, promote and facilitate support groups in underserved communities.
outreach to local community centers, urgent care facilities, and churches to
promote awareness of our services.
speaking engagements at community centers, libraries, etc. where we can educate
groups on our program and the services we offer.
in local events and fairs to educate the public on the services we offer.
with underserved caregivers, connecting them with services for both themselves
and the loved ones they care for.
follow-up support for the caregivers participating in our program.
out intake forms for all participants mandated by the Department of Health.
with other agencies to bring awareness of our program and its services and
ultimately create community partnerships.
Qualifications and Requirements include:
in Social Work or related field.
working with seniors a plus, not mandatory.
- 1-3 years of experience in direct counseling / experience
working with seniors a plus
- Bilingual Spanish a must.
NYS Driver's license with a safe and responsible driving history and a motor
vehicle that is properly registered and insured in accordance with NYS DMV
35 hours per week.
