Patient Navigator

Location: Hauppauge, NY | Schedule: Full Time, 35 Hour Per Week| Status: Non-Exempt |Salary:24.73/Hour




The Opportunity


The Patient Navigator will conduct field-based navigation and linkage to care to decrease the risk of acquiring HIV infection and to improve health outcomes for people living with HIV/AIDS.  The Patient Navigator facilitate Seeking Safety groups geared towards addressing impact of past trauma as well as conduct outreach and facilitate working relationships with community partners.


Responsibilities

Work closely with all FCA programs servicing individuals living with or at risk of HIV/AIDS, including THRIVE Recovery Centers, Family Treatment and Recovery Centers, Sherpa, and other support programs, to identify and enroll clients in need of health navigation services (HNS).

  • Conduct outreach at community locations, through collaborations with other organizations and/or through virtual platforms online serving target populations where individuals in need of HNS services are likely to be found.
  • In addition to ensuring all clients are linked to and remain engaged in medical care and support services per their action plans, the Care Coordinator will ensure HIV negative clients and HIV positive clients with a reported HIV negative partner are made aware of PrEP/PEP services and provide linkage to these services.
  • Document all services provided utilizing tool provided by Northwell CART.
  • Conduct the Evidence Based Intervention (EBI) Seeking Safety with clients one-on-one or in a group setting.
  • Promote and distribute condoms during each client level encounter with persons living with HIV/STIs or at high risk of contracting HIV/STIs. Providing condom education as needed when distributing condoms.
  • Cooperate with and contribute to all applicable service coordination and quality improvement initiatives agreed upon by consortium partners or recommended by the Program Advisory Board.
  • Submit service delivery data and other program data into designated data submission portal monthly.
  • Participate in monthly conference calls with Northwell CART Program Coordinator to review program progress, discuss challenges and quality improvement efforts.


  • Qualifications

    High school diploma or Equivalent required; Bachelors degree preferred.
  • A minimum of 6 months of experience required. 
  • Proficient computer skills with MS Office.
  • Excellent verbal and written communication skills.
  • Valid and clean driver's license.
