Veteran Outreach Navigator

Location: Garden City|Schedule: Full Time 35 hours per week, Monday-Friday| Status: Non-Exempt | Salary Range: 24.73-30.22/hour 

The Opportunity

The Veteran Outreach Navigator assists veterans and their families/caregivers in accessing healthcare and resources that impact their behavioral health. This role involves establishing connections with community-based veteran support services and providers to facilitate streamlined referrals to FCA services. The Navigator should possess a strong understanding of available veterans' services and benefits. Responsibilities include conducting outreach by attending community events, participating in coalitions, and connecting individuals with resources to facilitate referrals to FCA services and guide them to additional community-based services.

Responsibilities

  • Provide case management to veterans and their families/caregivers in-office and in the community using motivational interviewing
  • Conduct screenings, advocate for clients, and connect them with appropriate services and resources.
  • Collaborate with veteran service providers, advocates, and navigators to streamline care coordination, enhance service delivery, and support organizational initiatives addressing care-access barriers.
  • Engage in community outreach across Long Island to link veterans and families with FCA services.
  • Manage a caseload by developing, implementing, and monitoring treatment plans
  • Maintain timely, compliant electronic case records and complete all agency, program, and regulatory reports and documentation by required deadlines.
  • Participate in interdisciplinary team meetings, case conferences, workshops, and supervision.
  • Perform additional responsibilities to support the operations of the program. 

Qualifications

  • Bachelor's degree in the human service field required.
  • Minimum of one year of experience in the community health care setting and with working with veterans and their families.
  • Proficient computer skills with MS Office. 
  • Valid and clean driver's license required.
  • Bilingual in Spanish is preferred.
