Location: Wantagh| Schedule: Full-Time| Status: Non-Exempt | Salary: $24.73/Hour


The Opportunity

The Case Manager will provide crisis-orientated and preventive services to children and families experiencing difficulties at home to support improved family functioning and reduce family conflict.  

Responsibilities

• Conduct family and individual  counseling sessions.
• Provide case planning and case management services for clients, connecting them with various resources as needed.
• Assess youth's appropriateness for the program and provide appropriate referrals if necessary.
• Implement preventive interventions to avoid neglect, abuse or a runaway situation at home.
• Work collaboratively with the Family Mediator Case Manager to ensure the appropriate service plan is developed and implemented.
• Assist youth in returning home when appropriate.
  
• Additional responsibilities as needed to ensure the effective operation of the program

• Qualifications
  
  Master's Degree in Social Work, Marriage and Family Therapy, Mental Health Counseling or a related field required.
• A minimum of two years full time experience working with youth/young adults.

• Excellent verbal and written communication skills.
• Proficient computer skills with MS Office.
• Valid and clean driver's license.
