Program Coordinator

Location: Hempstead, NY | Salary: $55,000/year | Schedule: Full-time, 35 hours per week



The Opportunity

The Program Coordinator provides case management services and crisis intervention to teen parents and expecting teen parents.


Responsibilities

  •  Facilitate family life education groups and Prevention workshops.
    • Provide outreach and casework services, including informal counseling, advocacy, referral and information.
    • Provide crisis intervention and linkage to appropriate services.
    • Develop, implement and monitor case management goals.
    • Conduct Prevention workshops and webinars
    • Outreach to Mother/child residential programs, schools, and other community-based organizations.
    • Coordinate workshops with additional FCA programs to maximize services
    • Conduct home and field visits
    • Organize recreational/ socialization activities
    • Coordinate interagency collaboration and services to clients.
    • Complete all required reports, correspondence and other paperwork in accordance with Agency and regulatory requirement and submit by the required deadline.
    • Supervise PACT Case Manager, Interns and Volunteers

    Qualifications

    • Bachelor's Degree in Social Work or related field with appropriate experience.  MSW preferred.
    • Minimum of two years of experience working with adolescents and groups.
    • At least one year supervisory experience preferred.
    • Bi-lingual English-Spanish required.
