Case Manager

The Opportunity

The EISEP Senior Case Manager provides case management and ongoing monitoring services to seniors, 60 years and over, who reside in Nassau County. The Senior Case Manager has a BA or BS and 5 years of case management experience working with seniors in the field.

Qualifications

Required

  • College Degree in Social Work or related field.
  • Minimum of 5 years of case management experience required.
  • Excellent verbal and written communication skills.
  • Valid New York State Driver's License required.

Preferred

  • Experience with geriatric population preferred
  • Bilingual in Spanish preferred.

Additional Information

  • Non-Exempt, 35 hours per week
  • Location: Garden City
  • Salary: 26.21/hr
