Case Manager


Primary Responsibilities include:

  • Provide case management services and crisis intervention.
  • Complete intake and psychosocial assessments.
  • Development and implementation of case management goals.
  • Provide counseling, advocacy, referral and information; provide linkage to all services to support a successful service plan.
  • Facilitate Seeking Safety groups under the supervision of the Project Coordinator.
  • Coordination of interagency collaboration and service delivery.
  • Comply with all reporting and data collection requirements.

Qualifications and Requirements include:

-  BA required, MSW, MA, MS and other related degrees preferred.

-  Experience with substance abuse disorders required.

-  Candidate must have excellent problem solving skills and be a self-starter/team player

-  HIV Experience with HIV/AIDS population a plus.

-  Background in trauma informed education a plus.

-  Intermediate computer skills with MS Office.

-  Valid NYS driver's license with a safe and responsible driving record.

Schedule:

-  Full-time, 35 hours per week.

-  Monday through Friday.
