Primary Responsibilities include:
- Provide
Caregiver Family Support Services as established with the family and on
the child's Waiver Service Plan.
- Adhere
to principles of Individualized Care Model and base activities on the
needs, abilities and strengths of each child and family.
- Create
and implement Waiver Service Plans by State guidelines, including renewing
plans by deadlines.
- Document
visits and contacts with clients through progress notes that are submitted
within 24- 48 hours of visits.
- Maintain
communication with Care Managers regarding client progress.
- Adhere
to agency and NYS requirements including HIPAA policies, ethical
guidelines, safety protocols and OMIG compliance.
- Successfully
complete all mandated trainings.
Qualifications and Requirements include:
- High
school diploma, high school equivalency preferred or a State Education
Commencement Credential (e.g. SACC or CDOS) with related human service
experience.
- This is a per diem role
- Experience
working with children/youth preferred.
- Valid
New York State driver's license with a safe and responsible driving
history.
