Family Support Worker

Primary Responsibilities include:

  • Provide Caregiver Family Support Services as established with the family and on the child's Waiver Service Plan.
  • Adhere to principles of Individualized Care Model and base activities on the needs, abilities and strengths of each child and family.
  • Create and implement Waiver Service Plans by State guidelines, including renewing plans by deadlines.
  • Document visits and contacts with clients through progress notes that are submitted within 24- 48 hours of visits. 
  • Maintain communication with Care Managers regarding client progress.
  • Adhere to agency and NYS requirements including HIPAA policies, ethical guidelines, safety protocols and OMIG compliance.
  • Successfully complete all mandated trainings.

Qualifications and Requirements include:

  • High school diploma, high school equivalency preferred or a State Education Commencement Credential (e.g. SACC or CDOS) with related human service experience.
  • This is a per diem role
  • Experience working with children/youth preferred.
  • Valid New York State driver's license with a safe and responsible driving history.
