Counselor

III.  ESSENTIAL  DUTIES AND RESPONSIBILITIES:

  • Provide telehealth and in-home visitations to clients while collaborating with program staff to address client's needs.
  •  Prepare and maintain client case records (intake paperwork, behavioral health assessments, individualized care plans, discharge paperwork, etc.)
  •  Maintain and document statistics as required by the standards set by Family & Children's Association, New York State Office for Mental Health, & other grant guidelines.
  •   Provide monitoring and reassessment of assigned cases to assure that all services are being delivered as planned, and to measure changes in need.
  • Complete billable notes in the electronic health record system for counseling services.
  • Assist clients with completing forms/applications to apply for services & programs; provide information/referrals to clients as requested; address barriers to connecting with services/ resources.
  •   Participate in supervision, weekly team meetings; participate in agency trainings and workshops; and additional trainings/workshops that would be helpful to the position.
  • Work collaboratively with staff to deliver coordinated care and connect clients with services to address identified needs
  • Participate in outreach events within the community to offer education on mental health, SUD and increase awareness of program services.
  • Must drive own vehicle, which is properly insured and registered, to various worksites including the homes of clients.


Qualifications and Experience:

  • Master's degree in Social Work or related field required. (LMSW, LCSW or LMHC) 
  • MSW) or Licensed in related field.
  • Experience working with older adults with chronic medical conditions, mental health diagnosis, or substance use diagnosis.
  • Excellent organizational skills and ability to multi-task in a busy deadline, oriented work environment.
  • Must be able to maintain a high level of confidentiality.
  • Strong verbal and written communication skills 
  • Valid New York State driver's license with a safe and responsible driving history.  
  • Must drive own vehicle, which is properly insured and registered, to various worksites including the homes of clients.
  • Basic computer skills with Microsoft Office and the ability to readily learn new software required.
