Primary Responsibilities include:
individuals who are recently discharged from inpatient and/or detox service and
connect them to care within 14 days of discharge.
in discharge planning and/or meet individuals at the hospital when possible.
with scheduling an appointment at FCA's two outpatient clinics, providing the
necessary reminders and support to ensure the individual keeps their
appointment, and if needed, accompanying the individual to the appointment.
systems advocacy and guidance to individuals as they navigate the treatment
outreach to clinic clients that do not keep a scheduled appointment,
re-engaging them by exploring barriers to attending scheduled appointments,
helping individual to identify strengths that will allow them to be successful
in treatment, and accompanying individual to their appointment.
individuals to recovery supports and other community resources as needed.
other tasks as needed to meet overall program goals.
Qualifications and Requirements include:
School diploma required, Undergrad a plus.
- Completion of Recovery Coach Training Academy (minimum
Recovery Peer Adovate (CRPA or CRPA-P) required.
in delivery of peer recovery services and supports preferred.
- Proficient in Microsoft Office (Word, Outlook, Excel)
and the ability to readily adapt to program specific software.
