Intern will be making phone calls and outreaching to new clients to help determine eligibility and interest in case management services, accompanying Care Managers on visits, attending weekly SPOA meetings with Director, participating in outreach events, researching resources, and assisting in chart reviews.
Qualifications and Requirements include:
Junior or senior level undergraduate student seeking Social Work or Human Service Degree or 1st Year Master student.
Valid New York State driver's license with a safe and responsible driving history
Vehicle that is properly registered and insured by New York State and is available for use to conduct home/field visits
Schedule:
Opportunity available during the 2022/2023 Academic School Year (Fall 2022 and Spring 2023 Semesters)
14 to 21 Hours Weekly
Monday - Friday hours. Days are flexible
Blended Work Assignments (Both Remote and On-Site Work)
Supervisor Credentials:
LMSW, SIFI Certification
