Clinical Case Manager

Primary Responsibilities include:

· Provide a blend of telehealth and in-home visitations to clients while collaborating with program staff to address client's needs.

· Maintain client case records, assessments, and statistics as required by the standards set by Family & Children's Association, New York State Office for Mental Health, & ARP (CSLFRF) grant guidelines.

· Complete billable notes in an electronic health record system for counseling services.

· Provide case management services to clients such as: assisting clients with completing forms/applications to apply for services & programs; provide information/referrals to clients as requested; address barriers to connecting with services/ resources.

· Accompany clients to appointments to provide support and advocacy, when needed, for clients regarding entitlements and other basic needs; represent them at DSS, SSA, etc., when they are unable to represent themselves in-person or via telehealth.

· Participate in supervision with Program Coordinator and weekly Team Meetings; participate in agency trainings and workshops.

Qualifications, Requirements & Schedule include:

· Entry level position (experience with Geriatric or SMI population preferred)

· MSW or Masters in related field required 

LMSW or limited permit preferred

· Valid New York State driver's license with a safe and responsible driving history and a properly registered and insured vehicle in accordance with NYS-DMV regulations.

· Full time, 35 hours per week. Monday-Friday 9:00am-5:00pm

· Opportunity for partial remote work schedule- technology provided
