Non-Medicaid Care Manager
Location: Garden City NY | Schedule: Full-Time | Status: Non-Exempt | Salary Range: $24.45 / hour
The Opportunity
Are you passionate about making a meaningful difference in your community? We are seeking a dedicated Non-Medicaid Care Manager to join our team. In this role, you will provide essential advocacy, resource connection, and care coordination for individuals who do not qualify for Medicaid but still require vital support systems. You will empower clients to navigate complex social, medical, and community landscapes, ensuring they receive the high-quality care and stability they deserve.
Responsibilities
·Case Management: Conduct comprehensive assessments of client needs, strengths, and goals to develop personalized care plans.
·Resource Navigation: Connect clients and their families with community resources, alternative insurance options, housing assistance, and social services.
·Advocacy & Support: Act as a strong advocate for clients within medical, legal, and social service systems to ensure their voices are heard and needs are met.
·Monitoring & Evaluation: Maintain regular contact with clients to monitor progress, evaluate the effectiveness of care plans, and make adjustments as circumstances change.
·Documentation: Ensure accurate, timely, and confidential documentation of all client interactions, assessments, and care plans in compliance with organizational and regulatory standards.
·Collaboration: Partner with multidisciplinary teams, healthcare providers, and community organizations to build a seamless support network for clients.
Qualifications
EDUCATION:
·Bachelor's degree in Social Work, Psychology, Human Services, or a closely related field is required.
EXPERIENCE:
·Minimum of 2 years of experience in case management, care coordination, or social services.
·Valid, clean Driver's License and reliable personal transportation required for field visits and community travel.
·Strong knowledge of community resources and social service systems in New York.
·Exceptional communication, empathy, and interpersonal skills to work effectively with diverse populations.
·Excellent organizational skills and proficiency in electronic case management databases or record-keeping systems.
Compensation
The competitive hourly pay for this position is $24.45/hour. The competitive compensation range is reflected within FCA's framework, culture, and values. Individual pay decisions will vary based on skills and experience while still allowing for growth.
