The Opportunity

This per-diem role supports youth ages 5-21 who have a mental health diagnosis, or who are at risk, along with their families. The Intensive Mental Health (IMH) Worker meets with the youth three times a month to connect them to necessary supports and resources. Additionally, this position coordinates directly with schools and other treatment providers to ensure comprehensive continuity of care within Suffolk County.

After schoolwork hours: 3pm-7pm

Responsibilities

·Provide dedicated case management services and critical resource linkages to youth throughout Suffolk County.

·Conduct and maintain all strict visit requirements, ensuring exactly 3 in-person visits with the youth each month.

·Provide ongoing assessment and comprehensive documentation of the youth's evolving needs and progress towards goals.

·Link and refer families to specific services, supports, and community resources outlined in the youth's individual plan of care.

·Complete and submit all assessments, plans of care, progress notes, and other documentation on time and in compliance with agency and regulatory standards.

·Keep client charts continuously up to date and in strict accordance with program and funder guidelines.

·Maintain clear and transparent communication with all internal and external service providers.

·Perform routine assessments for safety and immediately inform the supervisor of any identified safety risks or incidents.

·Actively participate in all required agency training sessions and team meetings.

·Participate in community outreach and programmatic events as needed.

·Execute other tasks as assigned to support health and recovery operations.

Qualifications

EDUCATION:

·Bachelor's degree in a human services field required.

EXPERIENCE:

·Minimum of 2 years of relevant experience in the field.

·Direct experience working with children in a mental health setting is highly preferred.

WORK ENVIRONMENT & REQUIREMENTS:

·Valid New York State driver's license with a verified safe and responsible driving history.

·A reliable vehicle that is properly insured and registered in accordance with NYS DMV requirements, to be used for community visits.

·Intermediate computer skills with proficiency in Microsoft Office applications, including Word, Excel, and Outlook.
