FYI-Social Work Counselor

Location: Bohemia, NY | Schedule: FT 35 hours a week Evening and weekends as needed | Status: Non-Exempt |Salary Range: $30.22/hour

The Opportunity

Social Work counselor will work with individuals in the FYI-TPV program to provide supportive services to youth aging out of the foster care system and into independent living. All youth served by Foster Youth to Independence (FYI) Social Work Counselor are formally referred to by DSS and have been determined eligible for the Foster Youth to Independence Tenant Protection Voucher (FYI-TPV). Eligibility for FYI-TPV requires that youth be identified as at risk of homelessness and have a verified history of foster care involvement. DSS conducts the initial income and eligibility determination in accordance with HUD requirements prior to referral.

Responsibilities

· Conduct monthly home visits with 20 FYI youth.

· Complete and maintain client case records in a timely manner and in compliance with agency and other regulatory standards.

· Complete all required reports, correspondence and other paperwork and submit by requested deadlines.

· Participate in monthly planning meetings with Town of Brookhaven on each youth

· Help youth to develop decision making skills, personal responsibility, and sound judgment through structured instruction and hands on practice.

· Coordinate service efforts with Town of Brookhaven Voucher program regarding client's plan

· Provide support and referrals as necessary.

· Help clients build and sustain connections with adults and peers

· Recruit, assess and evaluate potential scholarship recipients

· Build linkages with community organizations and resources

· Attend conferences, trainings and workshops as appropriate

· Assume other tasks and responsibilities assigned to support the operations of the program

Qualifications

· Education: Bachelors degree in Human Service (required)/Masters degree in Human Service (preferred)

· Certificates or Licenses: NYS drivers licenses

· Experience: One or more years of experience working with adolescents
