Case Manager
Location: Garden City, NY | Schedule: FT Monday-Friday | Status: Non-Exempt |Salary Range: $26.92/hour
The Opportunity
The EISEP Case Manager provides case management and ongoing monitoring services to seniors, 60 years and over, who reside in Nassau County.
Responsibilities
- Provide an initial assessment of the senior adult's needs, develop a care plan for services and assist with the implementation of this plan.
- Provide monitoring and reassessment of all open EISEP case management cases to assure that all services are being delivered as planned, and to measure changes in need.
- Prepare reassessment forms for all EISEP clients receiving home care services; distribute EISEP forms to OFA and provider agencies; intervene in problems that arise between clients, aides and provider agencies.
- Assist with the EISEP Case Management Program office coverage biweekly and/or as needed - taking telephone intakes and seeing walk-ins who are requesting services.
- Provide information and referral to clients as requested.
- Provide advocacy, when needed, for clients regarding entitlements and other basic needs; represent them at DSS, SSA, etc., when they are unable to represent themselves.
- Participate in supervision with Program Supervisor; participate in agency trainings and workshops, OFA trainings and other county trainings, which would be helpful to case management.
- Prepare and maintain client case records in the database (Peer Place) and prepare and keep statistics as required by the standards set by OFA and FCA.
- Outreach within the community to provide information and assistance.
- Assume other tasks and responsibilities assigned to support the operations of the program.
Qualifications
·BSW/BA or related field
·1-3 years case management experience required, Experience with geriatric population preferred, bilingual a plus
·Valid New York State driver's license, car insurance with a safe and responsible driving history. A properly registered vehicle to regularly travel from worksite to home visits in Nassau County. Car is needed to visit clients in their homes.
