Patient Navigator (FT)

Location: Hauppauge, NY | Schedule: FT Mon-Friday with some weekends, | Status: Non-Exempt |Salary Range: $24.73/hour

The Opportunity

The Patient Navigator provides sexual health testing, education, and care linkage to reduce HIV, HCV, and STI transmission while addressing substance use and overdose risk. This role integrates testing with education on overdose recognition and response. They conduct HIV/HCV/STI testing, facilitate PrEP/PEP access, and connect clients to medical and recovery supports. Through site- and field-based outreach, they engage impacted Long Island communities to improve health outcomes.

Responsibilities

  • Work closely with all FCA programs serving individuals living with or at risk for HIV/AIDS, including THRIVE Recovery Centers, Clinical Services, Sherpa, and other support programs, to identify individuals at increased risk for HIV, HCV, and sexually transmitted infections (STIs).
  • Conduct outreach in community settings and through partnerships with local organizations, as well as virtual platforms, to engage target populations in need of sexual health and prevention services.
  • Provide rapid testing for HIV, HCV, and syphilis, and collect specimens for gonorrhea and chlamydia testing. Ensure appropriate referrals for oral and anal STI testing through community partners.
  • Facilitate timely linkage to medical care, treatment, and preventive services for individuals with positive test results or identified risk factors, including access to PrEP and PEP when appropriate.
  • Conduct comprehensive risk assessments addressing sexual health behaviors and substance use and develop individualized risk reduction plans using evidence based interventions such as motivational interviewing and stages of change.
  • Integrate overdose prevention education into all client interactions, including instruction on naloxone (Narcan) use, recognition of overdose, and safer substance use practices.
  • Promote and distribute prevention supplies and educational materials, including safer sex supplies (condoms, dental dams, personal lubricant) and public health prevention tools, with instruction on proper use in accordance with program guidelines.
  • Conduct educational workshops and presentations at FCA sites and community locations to increase awareness of HIV, HCV, STI prevention, and overdose prevention.
  • Document all services in FCA's electronic medical record (EMR) in a timely, accurate, and compliant manner in accordance with agency, state, and federal requirements.
  • Submit required program and service delivery data to funding sources, including OASAS and the New York State Department of Health, ensuring accuracy and adherence to reporting guidelines.
  • Participate in supervision, team meetings, and quality improvement activities to support program compliance, performance outcomes, and service integration.
  • Collaborate with internal staff and external partners to ensure coordinated, client centered care and continuity of services.
  • Perform additional duties as assigned by the Program Director.

  • Education: High School Diploma or Equivalent
  • Certificates of Licenses: N/A
  • Experience: At least 6 months of related experience and/or training, or an equivalent combination of education, experience, and lived experience relevant to HIV/AIDS, STIs, prevention (PrEP/PEP, usage of barriers), treatment navigation, substance use and overdose prevention services.
