Outreach Worker-SNUG

The Opportunity

We are seeking an Outreach Worker for our SNUG Program.  SNUG is an evidence-based violence reduction initiative that works with community residents, businesses and community-based organizations to implement strategies to reduce and prevent shootings and killings in communities with high rates of violent crimes.

Responsibilities

  • Conduct outreach and canvassing in the community (individually and as a team member).
  • Identify youth who are gang members and those at risk for joining gangs and intervening in their lives through case management.
  • Participate at daily briefs and debriefs with SNUG Outreach Worker Supervisor, Program Manager and OVS Social Worker which may include virtual means of technology.
  • Outreach Workers are expected to have a minimum caseload of seven to ten participants and meet face-to-face with each participant at least six times a month.
  • Document all their daily activities, including but not limited to contact with program participants, conflict mediation encounters, shooting responses, and community engagement events: barbecues, marches, rallies, etc.
  • Daily log must be completed in the DCJS database at the end of each shift.
  • Risk-Reduction Planning- to modify high-risk behaviors documented in the DCJS database every 90 days
  • Participate in organizing responses to shootings and increasing visibility when shootings take place to coordinate an inclusive and strategic response.
  • Attend weekly staff meetings which may include virtual means of technology.
  • Attend Participant case management meetings with OVS Social Worker
  • Serve as linkages and support for individuals to enhance their assistance and use of opportunities and programs in the community (e.g. job programs, GED, drug treatment, and mentoring).
  • Attend mandatory training and professional development training which may include virtual means of technology.
  • Follow all FCA and DCJS policy and procedures.

Qualifications

  • HS Diploma/GED 
  • Proficient with MS Office (Outlook, Word and Excel).
  • Lived experience and connections with at-risk Hempstead youth a plus.

Schedule:

  • Full Time, 35 hours per week.
  • Location: Hempstead
  • Salary: 22/hr
