Intensive Mental Health Provider (Part-Time)

Opportunity

We are seeking a P/T Intensive Mental Health (IMH) Provider for our Y-Fi Program.  This program supports Suffolk County families with children ages 5 -21 with a serious emotional disturbance or who are at-risk of being diagnosed.

Responsibilities

  • Conduct and maintain all visit requirements with child, family, and service providers.
  • Provide skill building services to clients as needed and in accordance with their service plan.
  • Provide on-going assessment and documentation of child and family's strengths and needs, progress towards achieving goals and efficacy of delivered services.
  • Coordinate and monitor Family Support and Youth Peer Advocacy Services, including attending monthly team meetings.
  • Link and refer to services and supports.
  • Complete and submit all reports, progress notes, service plans and any other written work on time and in compliance with agency and regulatory standards.
  • Keep client files up to date and properly filed in accordance with program and OMH guidelines.
  • Keep Coordinator appraised of the status of all cases and immediately inform them of any unusual situations in the family, including questionable CPS reports or any other safety concerns.
  • Continually operate from a strength based, family driven, and individualized care model.
  • Assume other tasks and responsibilities assigned to support the operations of the program.

QUALIFICATIONS 

  • Bachelor's Degree plus 2 years' experience providing direct services to children in the children's services system. Experience in the mental health field/working with children with SED preferred.
  • Valid New York State driver's license with a safe and responsible driving history. Must drive own vehicle, which is properly insured and registered, to various worksites including the homes of clients.
  • Intermediate computer skills with Microsoft Office, including Word and Excel.
