Case Manager $26.37/hr

The Senior Safety Net (SSN) program will increase access to care and address the broad care needs of seniors in Nassau County. SSN will target older adults 55 years old living in Nassau County that are at risk or have already presented with substance abuse and mental health challenges. SSN staff will conduct outreach and community education to raise awareness of behavioral health issues, dangers of misuse of prescription drugs, alcohol, substances and prevention of scams/fraud in the senior population. The program will offer short term supportive counseling, intensive case management, health education, and peer support through a holistic lens to address key social determinants of health by linking to resources and services.


Primary Responsibilities Include:

  • Provide telehealth and in-home visitations to clients while collaborating with program staff to address client's needs.
  •  Prepare and maintain client case records (intake paperwork, behavioral health assessments, individualized care plans, discharge paperwork, etc.)
  •  Maintain and document statistics as required by the standards set by Family & Children's Association, New York State Office for Mental Health, & other grant guidelines.
  •  Provide monitoring and reassessment of assigned case management cases to assure that all services are being delivered as planned, and to measure changes in need.
  • Accompany clients to appointments as needed to provide support and advocacy, when needed, for clients regarding entitlements and other basic needs; represent them at DSS, SSA, etc., when they are unable to represent themselves in-person or via telehealth.
  • Assist clients with completing forms/applications to apply for services & programs; provide information/referrals to clients as requested; address barriers to connecting with services/ resources.
  • Work collaboratively with staff to deliver coordinated services and connect clients with services to address identified needs
  • Assist and introduce clients in using technology to assist with maintaining good health, follow up with physician appointments and managing prescriptions.
  • Participate in outreach events within the community to offer education on mental health, aging services, and/or substance use while promoting program services.

Qualifications and Requirements include:

  • Bachelor's degree in Social Work or in a related field required. (BSW or related field) 
  • Experience working in case management program with older adults with chronic medical conditions or mental health diagnosis a plus.
  • Excellent verbal and written communication skills required.
  • Strong organizational skills and ability to multi-task in a busy deadline oriented work environment. 
  • Must be able to maintain a high level of confidentiality.
  • Bilingual Spanish a plus.
