Case Manager

Location: Hicksville NY | Salary: $24.73/Hour | Schedule: Full-time, 35 hours per week

The Opportunity

The Case Manager for the Families Healing Together program provides short-term counseling and case management services to youth ages 7-21 and heir families, along with community outreach and education throughout Nassau County to increase awareness of Adverse Childhood Experiences (ACEs) and strategies to reduce related risk factors. Services are delivered in office-based, community, and telehealth settings.

Responsibilities

  • Conduct community outreach and provide education on Adverse Childhood Experiences (ACEs), their long-term impact, and strategies to reduce risk factors.
  • Attend community events and deliver presentations to schools, community centers, and organizations to increase awareness of program services and referrals.
  • Provide individual and family case management services to connect clients with community resources and referrals to support their needs
  • Collaborate with internal programs and external service providers to support client progress and continuity of care.
  • Conduct intakes, complete assessments, and develop and monitor individualized care plans.
  • Participate in interdisciplinary team meetings, case conferences, and trainings.
  • Maintain accurate and timely client records, reports, and documentation in compliance with agency and regulatory requirements.
  • Perform additional duties and projects assigned to support program operations.

Qualifications

  • Minimum 1 year of experience working with youth, families, or in-community-based social services (internship experience will be accepted) required.
  • Bachelor's degree in Social Work or related field preferred.
  • Bilingual in Spanish and English preferred.
  • Knowledge of trauma-informed care, Adverse Childhood Experiences (ACEs), and behavioral health prevention preferred.
  • Ability to provide services in office, community, and telehealth settings.
  • Proficient computer skills with MS Office required.
