TEMPORARY JOB FOR 3-6 MONTHS
Primary Responsibilities include:
for intake and assessment of individuals seeking services for appropriate
level of care, utilizing approved screening and assessment tools,
for completion of assessment and admission documentation as dictated by
OASAS 822 regulations and the outpatient policy and procedures manual.
for the coordination of health assessments and psychiatric evaluations
with appropriate medical staff.
for explaining treatment process to participants, including the program
consent for treatment, program rules and regulations, confidentiality and
other consent forms. Responsible for making referrals to outside providers
a listing of referral agencies with contact names and phone numbers, and
maintains contact with these agencies for ongoing recruitment and referral
in weekly case conference meeting as well as weekly individual supervision
for assigning consumers to a Primary Clinician.

Qualifications and Requirements includes
degree in Human Service related major required; Master's degree preferred
Health Professional (QHP) preferred
year of experience working in in an OASAS funded facility preferred
with MS Office (Outlook, Word and Excel)
with Electronic Health Record (EHR) preferred
Spanish a plus
time, Monday- Friday; some evenings required
shared between Hicksville and Hempstead location
