Intake Coordinator-Temporary Opening!

Primary Responsibilities include:

  • Responsible for intake and assessment of individuals seeking services for appropriate level of care, utilizing approved screening and assessment tools, including LOCADTR.
  • Responsible for completion of assessment and admission documentation as dictated by OASAS 822 regulations and the outpatient policy and procedures manual.
  • Responsible for the coordination of health assessments and psychiatric evaluations with appropriate medical staff.
  • Responsible for explaining treatment process to participants, including the program consent for treatment, program rules and regulations, confidentiality and other consent forms. Responsible for making referrals to outside providers as necessary.
  • Maintains a listing of referral agencies with contact names and phone numbers, and maintains contact with these agencies for ongoing recruitment and referral purposes.
  • Participates in weekly case conference meeting as well as weekly individual supervision
  • Responsible for assigning consumers to a Primary Clinician.
  • Provides individual, group, and family counseling as needed.
  • Conducts all other assignments as given by the Program Director, or Clinical Supervisors.

Qualifications and Requirements includes

  • Bachelor's degree in Human Service related major required; Master's degree preferred
  • Qualified Health Professional (QHP) preferred
  • 1 year of experience working in in an OASAS funded facility preferred
  • Proficient with MS Office (Outlook, Word and Excel)
  • Experience with Electronic Health Record (EHR) preferred
  • Bi-Lingual Spanish a plus


  • Full time, Monday- Friday; some evenings required
  • Temporary, 3-6 months
  • Time shared between Hicksville and Hempstead location
