English/Spanish Bilingual Case Manager

Primary Responsibilities include:

  • Provide case management services and crisis intervention
  • Complete intake and psychosocial assessments
  • Development and implementation of case management goals via individualized service plan
  • Provide counseling, advocacy, referral and treatment education services
  • Facilitate linkage to all services to support service plan
  • Facilitate Seeking Safety groups
  • Coordination of interagency collaboration and service delivery
  • Comply with all reporting and data collection requirements
  • Other duties as needed

Qualifications and Requirements include:

  • Spanish/English Bilingual Speaking a must
  • BA, MSW, MA, MS and other related degrees preferred
  • Knowledge of local substance use, mental health and social service resources preferred
  • Experience with substance abuse disorders a plus
  • Experience with HIV/AIDS population a plus
  • Background in trauma informed education a plus
  • Intermediate computer skills with MS Office
  • Candidate must have excellent problem solving skills and be a self-starter/team player
  • Valid NYS driver's license with a safe and responsible driving record a plus

Schedule:

  • Full-time, 35 hours per week
  • Monday through Friday
