Care Coordinator

Primary Responsibilities include:

  • Complete care management assessments and monitor client progress towards improving management of co-occurring physical health, mental health, and substance use issues
  • Assure the provision of education, risk reduction, counseling and referral services to all clients regarding HIV/AIDS, tuberculosis, hepatitis, sexually transmitted diseases, and other communicable diseases. Administer Rapid HIV and conduct pre and posttest counseling; provide referrals when indicated, Refer to outside providers for Hepatitis C and B testing
  • Monitor service plans and update, in conjunction with clinician, to resolve barriers to service utilization
  • Ensure entitlements, insurance, and benefits are in place
  • Coordinate with primary care and mental health providers, as well as on-site clinical team
  • Coordinate with other facilities at all levels to ensure the continuity of care as clients transition between care systems.
  • Provide education & coaching for the client and their support system regarding medications, health behavior changes, and accessing community resources & services
  • Enter clinical data into electronic systems accurately & efficiently
  • Complete all documentation in a timely, client-centered manner
  • Provide referral to services that best meet client needs; advocate as needed for appropriate service provision; and follow up on outcome of referral, obtaining all necessary information from referred provider
  • Assist Medical Providers in the coordination of MAT services
  • Conduct client and community outreach as needed

Qualifications and Requirements include:

  • Bachelor's degree in social work, psychology or related field OR Certified Nursing Assistant with 1-2 years of relevant work experience in behavioral health field.
  • A valid New York State driver's license with a safe and responsible driving history. Own motor vehicle, properly insured and registered in accordance with NYS DMV regulations
  • Must have intermediate computer skills with Microsoft Office in order to complete all required paperwork and perform other responsibilities. Must have the ability to readily learn and adapt to program specific applications, including an EHR system.


  • F/T 35 hours per week, some evenings required
  • Split time between Hicksville and Hempstead location
